You can always call Driving Solutions to set up a free consultation, but you’ll get the most out of your conversation when you are prepared.

Please download, print, and complete these documents before meeting with  your driver safety consultant:

1. An MD order for an OT Driver Evaluation
This is required. The prescription should contain the driver’s name and reason for the evaluation, or a diagnosis. Or click here to download a copy of the MD Order which can be printed and forwarded to the MD for signature.

2. Driving History
Complete the Contact Sheet about driving habits and awareness. The driver being evaluated should complete the questionnaires without help from others…it’s all part of the evaluation.

3. Medical History
Provide a written list of medications taken and complete this Medical History Form.

4.Identification
Make a photocopy of your driver’s license.
